Host Registration Forms
Please be aware that the DNS Host Registration Forms have changed.
These forms can be used to register new hosts or modify existing hosts within the campus DNS records. To use these forms, you must first authenticate yourself as a valid and authorized user of the University of Chicago data network. To authenticate yourself, simply enter the forms by following the link below and enter your CNetID and CNetID password. If you do not have your CnetID information, please visit the account information page for further instructions.
New DNS Host Registration Forms
The Host Registration Forms can be used to:
- register a new host
- rename an existing host
- relocate an existing host
- update the record for an existing host
- delete an existing host
The forms can also be used to create and modify CNAME type DNS records:
- create a new CNAME type record
- repoint an existing CNAME type record
- convert an existing CNAME type record to a new host record
- delete an existing CNAME type record
