Cellular Services Order Forms
Please use the links below to access the appropriate form for your order request:
- Request Cellular Services - Order a new or upgraded cell phone, change your current voice plan, or cancel cellular services
- Modify Cellular Services - Add or change your current cellular features
- Request for xMail Services - Order xMail, the University's integrated email and calendaring service, accessible via Microsoft Outlook and Entourage and on the web
- Request for xMail and Mobile Services - Request xMail services along with a compatible mobile device
