Remote Call Forward Feature Activation Form
The remote call forward feature allows you to “remotely” activate or deactivate the forwarding of your calls from any other phone on or off campus. For rooms that have multiple students, the feature will call forward all calls to the phone number designated. Students will be responsible for billed call charges for calls forwarded off campus.

By submitting this form, I understand there is no charge for this feature but I will be responsible for all billed call charges for calls forwarded on or off campus. I also understand that this service is connected with the phone number in my dormitory room regardless of the number of students sharing the room. I will notify NSIT Student Services if I change dorm rooms. Any problems will be reported to NSIT Student Services at 4-6000.
